Application attachments
Attach a document to your application that demonstrates your need for individual study arrangements:

Attachment requirements
- A Kanta document (from the national health records archive) must include your personal details, date and location of the appointment, diagnosis and the healthcare provider.
- A statement concerning a permanent disability or illness is accepted if it was issued when you were 16 or older. Otherwise, the period of validity is assessed on a case-by-case basis.
- Do not send original documents — the university will destroy all copies once the recommendation has been issued.
Sending your application by secure email
Always send your application and attachments by secure email (preferably as PDF files). Never send health-related information by regular email.
Sending instructions
- Go to the secure email webpage (opens in a new tab).
- Enter your email address (in the form @student.uef.fi) and the verification code shown on the page.
- You will receive a link to your email that allows you to write a secure email message.
- Enter the email address of your faculty as the recipient (see below).
- Attach your application and supporting documents to the message and click Send.

If you are an Open University student, send your application to the faculty whose courses you are taking.
Network meeting
If your situation requires a broader assessment, or if you wish, a network meeting can be arranged. During the meeting:
- the arrangements you need and their duration are discussed
- the practical implementation of the arrangements is assessed
- any follow-up is agreed upon
Who can participate in the meeting?
- You
- A representative of your academic subject
- Specialists (e.g. a study psychologist or special education teacher)
- A support person of your choice, if you wish (this can be anyone you choose)
Notes may be taken during the meeting if necessary. The notes will record what was agreed upon, but will not include any information about your health.
